According to the CDC, more than one-third of U.S. adults have obesity, and another 70% ​is overweight. The Body Mass Index (BMI) is a simple index of weight to height ratio. Overweight is a BMI greater or equal to 25 and obesity is a BMI greater than or equal to 30. Overweight and obesity are linked to health complications like diabetes mellitus, high cholesterol, hypertension, heart disease, polycystic ovarian syndrome, female infertility, depression, and others. Good news, overweight, and obesity is treatable a change in behavior or lifestyle are the first and most important steps in moving towards weight loss. Weight loss can be achieved with a combination of Diet, Exercise and Weight Loss Enhancers.

CDC-Diabetes Prevention Classes

According to the CDC 1 in 3 American adults are affected by pre-diabetes. That is 86 millions of adults have pre-diabetes, and 9 out of 10 don't know they have it. Pre-diabetes means your blood glucose (sugar) levels are higher than normal, but not high enough to be diagnosed as diabetes. Your hemoglobin A1c or a fasting blood glucose. If your range of blood glucose is 100-125mg/dl or your A1c between 5.7 and 6.4, you are considered to have pre-diabetes. We offer a one-year in-person Diabetes Prevention Program with a lifestyle coach.

Program Criteria:

  • You are 18 years or older.
  • Hemoglobin A1c is between 5.7- 6.4.
  • Fasting glucose is 100-125mg/dl
  • Previously Gestational Diabetes
  • Do NOT have diabetes
  • Are NOT currently pregnant

Diabetes Self-Management Education (DSME)

30 Million Americans have Type 2 Diabetes Mellitus.  If you are one of the millions of people living with diabetes, we have good news to help you live a better quality of life.  Participating in a diabetes self-management education program (DSME).  Can help you learn the skills to manage your diabetes more effectively. 

     Our self-management education offers 1:1 or in a group setting.  We provide you with the tool necessary for you to take control of your diabetes, thus preventing or delaying the onset of an acute or chronic condition.  Education offered includes managing acute and chronic complications, self-monitoring (blood glucose training), insulin injection training. Healthy Eating. Physical Activity, medication management, problem-solving and healthy coping.

Program Criteria:

  • You must have Type 2 Diabetes
  • A referral from your provide is required

Medical Nutrition Therapy

The goal of nutrition counseling or medical nutrition therapy is to assess medical history, food allergies and preferences and current health status to prescribe changes to the diet, nutrition education and behavior change counseling for improved health and optimal nutrition. 

 Because there is a direct correlation between diet and diabetes management, medical nutrition therapy provided by a registered dietitian is a key component to your diabetes care.

Telephonic Chronic Disease Management: 

We offer monthly telephonic disease management and coaching to  Medicare beneficiaries with the following required elements:

● Multiple (two or more) chronic conditions expected to last at least 12 months, or until the death of the patient

● Chronic conditions place the patient at significant risk of death, acute exacerbation/ decompensation, or functional decline

● Comprehensive care plan established, implemented, revised, or monitored for chronic disease such as: 

- Asthma


- Congestive Heart Failure

- Dementia

- Diabetes

- Hypertension

- Hyperlipidemia

- Medication Adherence

- Stress Management

- Chronic Kidney Disease
